Your doctor’s AI notetaker may be making things up, Ontario audit finds

Ontario’s auditor general tested 20 government-approved AI medical scribe vendors and found every one had at least one accuracy or completeness issue.

Problems included hallucinated details, transcription mistakes, and missing mental health information from simulated doctor-patient conversations.

The audit raises patient safety concerns because doctors may rely on flawed notes for referrals, medication instructions, and care plans.

Officials warned that inaccurate AI-generated records could harm treatment outcomes if not better monitored and controlled.
